During the first quarter of 2026, Derko 2, LLC and Reg 2018 Irrevocable Trust U/A/D 1/1/18 were listed as the owners of Brookside Rehab & Nursing Center, according to the Centers for Medicare and Medicaid Services (CMS).

The facility operates for profit.

In that quarter, the nursing home received an overall CMS rating of 1, which falls below the Virginia statewide nursing home average of 3.0. CMS assesses these ratings based on health inspection results, staffing data, and the quality of care delivered to residents.

The data indicates Brookside Rehab & Nursing Center averaged 119.3 daily residents out of 130 available beds in the first quarter of 2026. The facility currently does not have hospital-based status.

Throughout the first quarter of 2026, Brookside Rehab & Nursing Center incurred $146,073 in fines over two incidents and faced two penalties.

There were two for-profit nursing home facilities in Fauquier County, Virginia.

U.S. Census Bureau estimates show the population of older adults continues to rise while younger age groups decrease. From 2020 to 2024, Americans age 65 and over grew by 13%, compared with a 1.7% decline in the under-18 population. The older adult population in the United States has surged over the past 100 years, climbing from 4.9 million in 1920 to more than 61 million by 2024.

A 2021 study found almost half of this demographic—about 28 million people—could require long-term services and supports, with projections indicating more than one third may eventually need care in a nursing home.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance, an insurance education organization. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
